The overarching objective of this project is to improve access to cardiology specialists by family physicians who are faced with assessing, diagnosing and treating complex patients that exhibit a variety of existing and/or potential cardiovascular disorders using innovative models of care delivery such as telemedicine.  This proposal will pilot test an innovative shared-care model of care delivery (the RACE model) that involves cooperation and collaboration among members of Providence Health Care’s (PHC) Department of Family and Community Medicine, and Division of Cardiology.  The future vision is to expand the scope of this shared care model to include other chronic diseases such as diabetes under the auspices of Vancouver Coastal Health’s (VCH) Care Connectivity framework.
